Abstract

Background: Exercise during pregnancy is widely recognized for its benefits to both the mother and fetus, including improved cardiovascular health, reduced risk of gestational diabetes, and enhanced mental well-being. However, the type and supervision of exercise-whether supervised or unsupervised-can impact its effectiveness. Objective: This literature review examines and compares the outcomes of supervised versus unsupervised exercise during pregnancy from a physiotherapist's perspective, exploring the evidence on their benefits and potential risks. Methods: A review of recent studies evaluating supervised and unsupervised prenatal exercise programs was conducted. Emphasis was placed on maternal weight gain, birth outcomes, and the incidence of complications such as macrosomia, gestational diabetes, hypertension, and preterm birth. Results: Studies indicate that women following structured exercise programs gained weight within recommended guidelines, leading to improved birth outcomes and lower risks of complications like macrosomia and caesarean delivery. Research also highlights that prenatal physical activity reduces the risk of macrosomia without increasing the likelihood of smaller newborns, debunking concerns about reduced energy reserves for fetal growth in active pregnancies. Conclusion: Supervised exercise, with personalized guidance, ensures optimal intensity and technique, reducing risks such as gestational diabetes, hypertension, and preterm birth, making it essential for pregnant women, especially those with complications.
