Abstract

Frailty represents a particularly challenging aspect of population aging. While international guidelines advocate routine identification to enable evidence-based interventions, existing instruments often demand additional resources, limiting their practical implementation. Electronic tools facilitate the analysis of larger patient populations in less time, optimizing both resources and secondary data utilization. This project aimed to identify frailty using data from electronic medical records in diabetic patients aged 65 years and older. The Electronic Screening Index of Frailty (e-SIF), developed by Serra-Prat et al., was used in this study. This report outlines lessons learned from our experience and the processes involved in implementing this frailty detection tool.
