Abstract

Chronic kidney disease care remains fragmented, neglecting contextual whole-person health needs. We propose BeWell360-CKD care model, integrating Lifestyle and Integrative Medicine modalities with Minimally Disruptive Medicine frameworks to align care with patient capacity; strengthen activation, self-efficacy, and resilience; and reduce care burden through redesigned teams, digital health, and policy reform.
