Abstract
objectiveTo analyze, in primary care, the dependency and probability relationship between estimated body composition (adiposity-lean mass) and insulin resistance (IR) with hemodynamic variables.
designCross-sectional, observational, and descriptive study.
settingPrimary Care Service of Malgrat-Palafolls, Barcelona, Spain.
participants492 patients attended and included according to criteria (203 men and 289 women).
interventionsNone. MAIN MEASUREMENTS: Weight, height, and waist circumference were recorded to calculate body mass index, adiposity (GC%) using the Palafolls and CUN-BAE formulas, and lean mass using MCM% and James. Blood glucose, insulin, and HOMA-IR were measured, as well as systolic, diastolic, and mean blood pressure, and heart rate.
resultsThe dependency analysis between BMI and hemodynamic variables showed no mediation by IR (β=0.25-1.20, p=not significant). GC% did show mediation (β=6.25-9.98, p<0.05), providing a more precise characterization of the pathophysiological state. MCM% reproduced this relationship with inverse values (β=[-0.22-0.24], p<0.05), reflecting the implications of IR. In logistic models, GC% showed a higher probability of IR in the presence of elevated hemodynamic variables (OR=1.07 [1.04-1.10], p<0.05), while MCM% stood out with values below one (OR=0.94 [0.92-0.97], p<0.05), suggesting a possible protective effect against IR in scenarios of high blood pressure and heart rate.
conclusionsBody composition estimates based on GC% and MCM% allow the identification of hemodynamic repercussions of adiposity and the mediation of IR in clinically silent phases. MCM% suggests a protective effect under these conditions. Their simplicity of calculation places them above traditional formulas.
