Abstract
To identify the most effective exercise interventions for improving metabolic outcomes in individuals with metabolic syndrome, we conducted a network meta-analysis comparing the effects of ten different exercise interventions. A total of 53 studies involving 2,948 participants were included. Combined aerobic and resistance training (AT + RT) ranked highest for improving body mass index (BMI), waist circumference (WC), low-density lipoprotein cholesterol (LDL), and fasting blood glucose (FBG). High-volume high-intensity interval training (HV-HIIT) was most effective for reducing body fat percentage (FAT%), whereas low-volume HIIT (LV-HIIT) showed the greatest benefits for total cholesterol (TC), triglycerides (TG), and diastolic blood pressure (DBP). Traditional Chinese exercise (TCE) ranked highest for increasing high-density lipoprotein cholesterol (HDL) and reducing systolic blood pressure (SBP). Most evidence was rated as low or very low certainty. No single exercise modality was superior across all components of metabolic syndrome. These findings support a phenotype-tailored exercise prescription approach based on patients' primary metabolic abnormalities.
